Athletic Physicals: Ottawa Family Physicians will offer sports physicals on July 30 from 6:30 - 8:30 pm. Bring your completed physical form and $20. Physical forms will be mailed with enrollment forms later this summer or may be downloaded by clicking here.
All athletes who participate in summer practices must pay a $3.00 insurance fee per sport. Athletes who are using the weight room only (not participating in a summer sports' practice) must also pay $3.00.
